Information you give us
Our appointment, contact, new patient and nutrition consultation forms ask for your name and, depending on the form, your email address, phone number and a message. What you submit is stored in this website's database and emailed to the clinic. We use it only to reply to you and to schedule and prepare for your visit. We do not sell, rent or trade it.
These forms are not a secure patient portal. Please do not send medical records, diagnoses or other sensitive health details through the website. Call us at 503-472-6550 instead.
Information collected automatically
We use Google Analytics to understand how the site is used. It and similar tools set cookies in your browser and record things like the pages you view, roughly where in the world you are, which device and browser you use, and how you arrived at the site. We read this as totals and trends. We do not use it to work out who you are.
Third-party content on this site
Some pages embed content served by other companies: a Google Map on our contact page and patient video testimonials hosted on YouTube. When one of those loads, that company can set its own cookies and collect its own data under its own privacy policy. We do not control what they collect.
Text messages
If you give us your phone number on a form, you consent to recurring automated text messages as described on our Opt-In Consent page. You may reply STOP at any time to opt out.
Staff accounts
Clinic staff sign in to a private admin area to publish content. Those sign-ins use session cookies. Visitors to the public site do not have accounts and do not sign in.
Children's privacy
This website is not directed to children under 13, and we do not knowingly collect information from them. If you believe a child has submitted information through this site, contact us and we will remove it.
Your choices
You can block or delete cookies in your browser settings, and Google offers a browser add-on that opts you out of Google Analytics. You can reply STOP to any text message from us. If you want us to delete something you submitted through a form, call or email us and we will.

If our practices change, we will update this page and change the date at the top. We recommend checking back from time to time.
